Individuals with Cognitive Disabilities
We work with individuals who have intellectual disabilities, traumatic brain injuries, developmental disabilities, or other conditions that affect thinking, learning, or daily functioning.
Individuals with Mental Health Conditions
We support people living with depression, anxiety, bipolar disorder, schizophrenia, and other mental health conditions — helping them manage daily life and reach their personal goals.
Families & Caregivers
We know that caring for a loved one can be challenging. We work with families to provide guidance, resources, and relief so that caregivers can also take care of themselves.
Referring Providers
We partner with hospitals, clinics, social workers, and case managers who are looking for trusted community-based support services for their clients.
We See the Whole Person
We do not define people by their diagnosis. We start by getting to know each individual — their interests, their strengths, and their goals. Then we build a plan that makes sense for them.
We use plain language and take the time to make sure everyone understands their options. If you have questions, we will keep talking until things are clear.

Making a Referral
If you are a healthcare provider, social worker, or case manager who would like to refer a client, we make the process simple.
You can refer a client by contacting us directly. We will follow up within [X business days] to begin the intake conversation.
We accept referrals from:
- Hospitals and discharge planners
- Mental health clinics and outpatient programs
- Social services agencies
- Primary care providers
- Family members seeking help for a loved one
What We Need to Get Started
- Basic contact information for the individual
- A brief description of needs or goals
- Any relevant existing care plans (optional)
- Preferred contact method
We do not collect Protected Health Information (PHI) through this website. Detailed clinical information is exchanged through secure channels after initial contact.
